Telehealth involves the delivery of healthcare services using electronic communications, information technology, or other electronic means between a health care practitioner and a patient who are not in the same physical location. Telehealth services may include telemedicine consultations, medical evaluations, laboratory monitoring, and prescription management.
2. Independent Provider Relationship
By choosing to utilize telehealth services enabled through Your NovaPath, you acknowledge that medical services are provided directly by independent licensed physicians, nurse practitioners, or physician assistants affiliated with third-party medical groups. Your NovaPath provides administrative and technological platform support and does not exercise control over clinical decisions.
3. Benefits & Risks
Benefits: Convenient access to healthcare providers without traveling, efficient ongoing support, and personalized evaluation.
Potential Risks: Delays in evaluation due to technical limitations, incomplete medical history provided by the patient, or the need for in-person medical examination when indicated by clinical judgment.
4. Patient Rights & Acknowledgments
You have the right to withhold or withdraw consent to telehealth care at any time without affecting your right to future care or treatment. If you require emergency care, you must immediately contact 911 or go to the nearest emergency medical facility.
